To avoid delays, please coordinate with your HMO provider or click on the following link: https://www.kaiserhealthgroup.com/ to secure a Letter of Authorization (LOA) or Letter of Guarantee (LOG) – as well as other related documents – prior to your checkup.
Patients may also secure a LOA/LOG by visiting our HMO Center, which is located at the 4th Floor of the Main Building, or by calling 8254-1111 ext. 4066 / 4068 / 5041 / 5042 during operating hours. Our HMO Center is open Mondays to Saturdays (except holidays) from 8:00 AM to 5:00 PM.
